The main recognized risks of lentiviral HSC gene therapy are (1) insertional risk: the vector integrates into the genome, and in rare cases in the wider field, integration has been linked to abnormal cell growth; (2) conditioning risk: the chemotherapy used to make room for corrected cells carries its own side effects; and (3) procedure risk from stem cell collection and re-infusion. In FACTS, insertion-site analysis of all five patients showed a polyclonal profile with no clonal dominance, and there were zero product-attributable serious adverse events. Conditioning was non-myeloablative (melphalan-based), and 4 of 5 patients went home the same day.

Interpretation: The FACTS safety profile is encouraging, and the gentler conditioning regimen was a deliberate design choice to lower risk and burden.

Uncertainty: Five patients is a small safety database. Rare events may not show up until more patients are treated and followed for longer, which is why long-term monitoring is built into gene therapy development.

Safety questions are welcome below; we'd rather answer them in public.
